A standard cholesterol test measures how much cholesterol is being carried, not how many particles are carrying it. Those can differ, and particle number tracks cardiovascular risk more closely than cholesterol content does. This test uses ion mobility to separate lipoproteins by size and count them directly, reporting LDL particle number, LDL size and pattern, medium and small LDL particles, and large HDL particles. Smaller, denser LDL particles are associated with higher risk than larger ones at the same cholesterol level. It is generally used to add detail when a standard lipid panel and the overall risk picture do not align.
