Insulin-like growth factor 1 is made mainly by the liver in response to growth hormone, and it carries out many of growth hormone's effects on tissue. Its advantage as a test is stability: growth hormone is released in pulses and varies enormously through the day, while IGF-1 stays relatively constant, so a single sample is meaningful. It is used when assessing growth hormone deficiency or excess, including acromegaly, and in monitoring treatment. Levels fall naturally with age and are also affected by nutrition, liver disease and diabetes, which is why an age-adjusted z-score is reported alongside the raw value.
