Myeloperoxidase is an enzyme released by neutrophils and monocytes as part of the inflammatory response. Within artery walls it contributes to oxidative damage and to processes associated with plaque becoming unstable, which is the basis for its use as a cardiovascular marker. Because it reflects active inflammation rather than the lipid burden itself, it is used to add information when standard lipid results and overall risk assessment leave uncertainty, not as a first-line test. Levels rise with infection and other inflammatory conditions, so results are interpreted in that context rather than in isolation.
