A standard cholesterol test measures the cholesterol carried by lipoproteins rather than the particles themselves, and the two can diverge. This test uses nuclear magnetic resonance spectroscopy, which identifies lipoprotein particles by the signal they produce, to count them directly and measure their size. It reports LDL particle number, LDL size, small LDL particles, total and large HDL particles, HDL size, large VLDL particles and VLDL size. Particle number tracks cardiovascular risk more closely than cholesterol content, and the discrepancy is largest in people with high triglycerides, diabetes or metabolic syndrome.
