Testosterone is a hormone produced by both men and women, in substantially different amounts. It contributes to muscle and bone, red blood cell production and sexual function. This test reports total testosterone, meaning the protein-bound portion together with the small unbound fraction. It is measured using mass spectrometry, a laboratory method that identifies and quantifies substances by their molecular weight. Testosterone levels vary through the day and are generally highest in the morning, so the timing of the sample affects the result. A single measurement does not establish a testosterone disorder: results are interpreted alongside symptoms and are usually confirmed with repeat testing.
- For males: low testosterone is assessed using repeated morning measurements together with symptoms, rather than from one result.
- For females: testing is more often used to investigate higher than expected levels, which may be associated with irregular periods, acne or excess hair growth.
